Medical Case Management
The goal for Medical Case Management is to identify areas of patient need and through a system of referrals, link patients to the resources or services required. Our experienced Medical Case Managers provide access for our patients to essential medical care, medication and support services (such as mental health, substance abuse treatment, housing, transportation, grocery delivery and legal services). Eligibility requirements may apply and waiting lists are possible.
During the assessment, the Medical Case Manager will work with the patient to develop an individualized service care plan and track the patient’s progress toward meeting established goals.
Self Managed Patient
Patients can be assessed as self managed by their Medical Case Managers. These patients do not have critical needs and can address issues themselves with minimal help. If a Self Managed patient has a concern they can contact the resource line:firstname.lastname@example.org or by calling the resource line which is checked 3 x a day, Monday through Friday at 678-990-6413.
The Patient Navigator is a contracted position funded by Ryan White Part A. The Patient Navigator assists patients with information and education regarding access to healthcare and the patient resources that are available. The Patient Navigator can provide regular contact with newly diagnosed patients through the initial medical service enrollment period and can offer emotional support and make referrals if needed, to support groups and workshops.

- Provide verification of HIV status – antibody test only one time
- Provide Verification of Income every six months
- Provide Proof of residency every six months
- Call to cancel appointments 24 hrs in advance
