Programs
Having stable housing is an important step in becoming and staying healthy. We currently offer several housing programs to help clients keep their housing and prevent homelessness. Services provided include rent, utility, mortgage, emergency hotel lodging, and move-in cost assistance. Staff will also provide a resource list and referrals to other agencies in the area.
Maximum Assistance Limits
- $4200 –Â Rent/Utility/Mortgage Assistance
- $2000 – Move-In Assistance (deposits, administrative fees, and first month’s rent)Â
- 90-day Hotel Stay
Eligibility Requirements
- Current PIHC patient currently being seen by the clinic,Â
prevention, or the emotional wellness and recovery team. - If you are a new patient, you must have completed at least 1 visit and remained engaged in care for 30 days before applying.
- Completion of all PIHC’s consent documents
- You must have income or be able to show proof of future income
- Undocumented individuals are eligible to apply
- Priority for hotel lodging assistance will be given to medically fragile patients, families with children, those fleeing domestic violence and cold weather assistance. Medically Fragile is defined as someone who has an acute medical
issue that would be made worse by being homeless.
